In 2014, a Dutch man let researchers inject him with a bacterial toxin that normally leaves healthy people shivering, feverish, and miserable for hours. He breathed in a particular pattern, and his body barely reacted. Then twelve people he had trained for ten days did the same thing — and as a group, they had milder symptoms and a measurably calmer immune response than untrained controls. The man was Wim Hof, and that PNAS study is the single best piece of evidence that the Wim Hof Method does something real. It is also where most honest accounts of the method should begin and end their hype.

The Wim Hof Method gets sold two ways. One crowd treats it as a superpower that cures disease and beats cold with the mind. The other dismisses it as a shirtless man hyperventilating in the snow. The truth is narrower and more useful than either: it is a structured way to expose yourself to controlled stress — cold and breath — and there is genuine science under part of it, real danger in one corner of it, and a lot of marketing in between. This is the honest version.
What it actually is
The Wim Hof Method is a named, packaged practice for deliberately stressing your body in controlled doses and learning to stay calm inside that stress. Strip away the branding and it is a modern descendant of something old: meeting hardship on purpose, in a measured amount, so it has less hold over you. If you've read about voluntary discomfort, you already know the family this belongs to. Wim Hof simply built a repeatable system around two of its oldest tools — cold and breath — and added a third he calls mindset.
What makes it distinct is the breathing. Plenty of people take cold showers. The method's signature is pairing the cold with a specific cycle of deep over-breathing and breath-holds, then claiming the combination lets you reach the autonomic nervous system — the part of you that runs heart rate, immune response, and body temperature on autopilot. That claim is the interesting one, and it's the one worth examining carefully.
The three pillars
Wim Hof teaches the method as three pillars that reinforce each other. Skipping one doesn't break it, but the system is designed to be practiced together.
- Breathing.Rounds of roughly 30 to 40 deep breaths followed by a breath-hold on an empty exhale, repeated three or four times. This is the engine of the method and, as we'll see, the part with both the most physiological effect and the most risk.
- Cold exposure. Gradual, progressive cold — usually starting with a cold finish to a warm shower and building toward longer cold showers and ice baths. The cold trains your body to handle a sharp stressor and teaches you to stay slow and steady while your nervous system screams.
- Commitment and concentration. The least glamorous pillar and arguably the real one. The breathing and the cold are useless if you negotiate with yourself every morning. The mindset piece is about doing the hard thing without bargaining — the same self-trust that any honest discomfort practice is really after.
What the breathing does to you
Here is the part most enthusiastic explainers skip, and it's the part you most need. The breathing feels mystical. The mechanism is not. A standard round looks like this:
- Take 30 to 40 deep breaths — full inhale, relaxed exhale that you don't force all the way out.
- After the last exhale, stop and hold with empty lungs for as long as is comfortable.
- When you need to, take one big breath in and hold it for about 15 seconds.
- Let go, and start the next round. Three to four rounds total.
That deep, fast breathing is controlled hyperventilation. It blows off carbon dioxide faster than your body makes it, which drops the CO₂ in your blood and temporarily raises your blood's pH — a state called respiratory alkalosis. Low CO₂ is the reason you can hold your breath far longer than feels possible after a round: the urge to breathe is driven mostly by rising CO₂, not falling oxygen, so by clearing the CO₂ you mute the alarm. The breathing also triggers a short surge of adrenaline, which is why people feel buzzy, tingly, and lightheaded.
None of this is supernatural, and understanding it is exactly what keeps you safe. The same low-CO₂ state that lets you hold your breath also lets you faint without warning, because you've quieted the alarm that would normally make you gasp. On a couch, fainting means you slump over. Anywhere else, it's the whole danger of the method — which we'll get to.
What the science actually shows
This is where you should slow down, because the gap between what's been shown and what gets claimed is large.
The immune study is real and genuinely striking. In the 2014 PNAS paper by Kox and colleagues, twelve people trained in the method for ten days were injected with an E. coliendotoxin alongside untrained controls. The trained group produced more adrenaline, more of the anti-inflammatory signal IL-10, fewer pro-inflammatory cytokines, and reported fewer flu-like symptoms. That is a real, peer-reviewed result showing healthy people voluntarily influencing an immune response that's supposed to be automatic.
Practitioners showed a profound increase in plasma epinephrine, attenuated the innate immune response, and reported fewer flu-like symptoms than untrained controls.
But read the fine print.Twelve trained subjects, all young healthy men, one ten-day training block, one toxin challenge. It shows the method can shift the response. It does not show it cures or prevents any actual disease, and it has not been widely replicated in different populations. Adrenaline transiently calming inflammation is interesting; it's not a treatment.
The brain-imaging work is even thinner. The 2018 NeuroImage study often cited as proof that Wim Hof can "control his body temperature with his mind" was centered on Wim Hof himself. It suggested the forced breathing helped generate heat and engaged brain regions tied to suppressing pain and stress. That's a fascinating case study of one extraordinary person. It tells you almost nothing about what will happen to you.
The cold piece stands more on its own. A 2016 randomized trial of cold showering — not the full method — found people who finished their showers cold took about 29% fewer sick days, though cold-exposure research overall is still mixed and self-reported. The one rule that has killed people
Most of the method is reasonably safe for a healthy adult on dry land. There is one exception, and it is not a small one. People have drowned doing the Wim Hof breathing in or near water, and the method's own creators warn against it in plain language.
The mechanism is the same physiology from earlier. The breathing clears CO₂ and quiets your urge to breathe. That lets you hold your breath far longer than normal — but oxygen is still draining the whole time. Because the CO₂ alarm is muted, you can pass out from low oxygen with no warning and no gasp first. This is called a hypoxic or shallow-water blackout. On land you slump. In water you go under silently and don't come back up.
Never practice the breathing before or during swimming, diving, or in any body of water. The breath-holds can cause you to faint.
Take that as an absolute, not a suggestion. Not in a pool. Not floating in the sea. Not in the bathtub. Not in the shower if you'd be doing the breath-holds standing on a slick surface where a faint means a fall. The standard, safe way to do the breathing is sitting or lying down somewhere you can't hurt yourself if you black out. How to try it safely this week
If you want to test the method honestly, here is the version that respects both the upside and the risk. Start smaller than you think you need to.
- Separate the two practices. Do the breathing in the morning, sitting or lying on a bed or couch. Do the cold later, in the shower, breathing normally. Never on the same clock, never in the same place.
- Do one or two rounds, not four. For the first week, try a single round of about 30 breaths and a comfortable empty-lung hold. Stop if you feel more than mildly lightheaded. You are learning the pattern, not setting a record.

Who should be careful
The method is meant to make you steadier, not to send you to a hospital. Some people should modify it heavily or skip it entirely, and the breathing is the riskier half for most of them.
- Heart and blood-pressure conditions.Both the breathing's adrenaline surge and the cold's sharp spike in heart rate and blood pressure are real stressors. If you have heart disease, very high blood pressure, or a history of cardiac events, talk to a doctor first.
- Epilepsy or a history of fainting. Controlled hyperventilation can provoke seizures or blackouts in susceptible people. This is a talk-to-a-professional situation, not a willpower one.
- Pregnancy. The breath-holds and the cold shock are not worth the unknowns. Skip the breathing and the ice baths unless a doctor clears you.
- Anyone, near water.This isn't a special-population caveat — it's for everyone. The single most dangerous thing about the method is doing the breathing where a faint could drown you.
That's the whole honest picture. The Wim Hof Method is a real practice with one genuinely impressive study, a pile of smaller and shakier ones, a modest case for the cold, and a single rule that's worth more than all the rest combined: do the breathing on dry land, alone with yourself, and let the steadiness — not the spectacle — be the reason you keep going.
